AS A CONDITION OF PRIZE ACCEPTANCE, WINNERS WILL BE REQUIRED TO PROVIDE AND/OR COMPLETE AN AFFIDAVIT OF ELIGIBILITY, TAX INFORMATION, RELEASE OF LIABILITY, AND PRIZE ACCEPTANCE AGREEMENT, AS REQUESTED BY SPONSOR.

BY ENTERING (OR OTHERWISE PARTICIPATING) IN THE SWEEPSTAKES, YOU AGREE TO THESE OFFICIAL RULES, WHICH CREATE A CONTRACT, SO READ THEM CAREFULLY BEFORE ENTERING. WITHOUT LIMITATION, SUCH CONTRACT INCLUDES A CLASS ACTION WAIVER AND A LIMITATION OF YOUR RIGHTS AND REMEDIES.

ELIGIBILITY: Versiti’s Let’s Party in style sweepstakes entry (“Sweepstakes”) is open only to legal U.S. residents of Michigan, Ohio, Illinois, Indiana, and Wisconsin who are 18 years of age or older at the time of entry and have reached the age of majority in their respective states of residence. The following are not eligible: (i) employees, officers, directors, agents, independent contractors, and representatives of Sponsor and its respective parents, affiliates, subsidiaries, wholesalers, retailers, distributers, advertising, and promotion agencies; (ii) employees, officers, directors, agents, or representatives associated with entities that prohibit the acceptance of items from Versiti involving amounts equal to the approximate retail values of the prizes identified below; and (iii) the immediate family members (mother, father, sister, brother, husband, wife, daughter, son, step-father, step-mother, step-daughter, step-son, step-brother, step-sister, half-brother, half-sister) and/or those living in the same household of any persons under (i) and (ii) above. The Sweepstakes is subject to all applicable federal, state, and local laws and regulations. Void where prohibited or restricted by law or regulations. Participation constitutes entrant’s full and unconditional agreement to these terms and conditions and Sponsor’s decisions, which are final and binding in all matters related to the Sweepstakes.

SWEEPSTAKES PERIOD: The Sweepstakes begins at 12:01 a.m. Central (“CT”) on January 1st, 2024, and ends at 11:59 p.m. (CT) on December 31st, 2024.

HOW TO ENTER: There are two ways to enter the Sweepstakes: (1) by donating or attempting to donate at a Versiti Blood Center donor center or mobile blood drive located in Wisconsin, Michigan, Ohio, Indiana, or Illinois during the Sweepstakes Period; or (2) by submitting an online Sweepstakes entry during the Sweepstakes Period. The choice of entry method will not affect an entrant’s chances of winning.

1. To enter by making six (6) donations of O-negative blood: Donate or attempt to donate six (6) O-negative blood at a Versiti Blood Center donor center or mobile drive (i) between January 1st, 2024, and ends at 11:59 p.m. (CT) on December 31st, 2024. For locations of Versiti Blood Centers and mobile drives, please visit Versiti.org. Donors will be tracked by Sponsor, and those individuals who make a donation of O- negative blood type during the stated period above will automatically receive one (1) entry into the drawing. To enter via this entry method, individuals must have O-negative blood type and must meet the minimum qualifications to donate, which can be found at Versiti.org.
2. To enter without donating (“AMOE”): Eligible entrants may enter the Sweepstakes without a blood donation by doing the following: Visit Versiti.org/Onegsweeps and fill out the entry form online during the sweepstakes Period. By completing the online form, you will receive one entry into the Sweepstakes.

Limit one (1) entry into the Sweepstakes, regardless of method of entry. Any attempt by an entrant to obtain more than the stated number of entries by using multiple/different addresses or any other methods will void that entrant’s entries and that entrant may be disqualified from the Sweepstakes. Entries submitted by anyone other than the entrant are void. Sponsor will not verify receipt of entries for entrants. All entries submitted become the sole property of Sponsor and will not be returned. Sponsor is not responsible for lost, late, misdirected, illegible, damaged, or postage-due mailed entries.

ODDS OF WINNING, WINNER SELECTION, AND NOTIFICATION: Odds of winning depend on the number of eligible entries received. The potential winner(s) will be selected randomly on or about April 18th, 2025. Winner will be notified via phone and/or email on or about May 1, 2025 (using information in Sponsor’s records or information provided via the mail-in entry) to verify his/her eligibility and ability to accept the prize, and to obtain his/her complete contact information. Any such message will instruct the winner to reply as specified within five (5) full business days (measured in the local time zone) after the time that the message was left or sent. Winner must reply as instructed to identify him/herself, confirm his/her eligibility and ability to accept the prize, and provide his/her complete contact information within such five (5) full business day period, or he/she will be disqualified. Potential winner will subsequently be required to complete an affidavit/certification of eligibility, a liability release, and (where legal) a publicity release, which must be returned within the reasonable time period specified by Sponsor at the time of such request. Winner will also be required to return a properly completed IRS W-9 form within the time period specified by Sponsor, and each such winner will receive an IRS Form 1099 issued in his/her name for the fair market value of prize received. Proof of identity, age, and legal residence may be required to claim a prize. Winner may be disqualified in Sponsor’s sole discretion if: (a) he/she fails to reply or to provide any required information or properly completed documents within the required time period; (b) he/she fails to comply with any of the terms of these Official Rules; or (c) if any prize or prize notification is returned as undeliverable. In the event of disqualification of the winner, at Sponsor’s sole discretion the prize may be awarded to an alternate winner selected from among the remaining applicable eligible entries via the means of winner determination described herein, pending verification of eligibility. If any prize remains unclaimed after three (3) potential winners have been notified for that prize, the prize may remain unawarded, in Sponsor’s sole discretion. No responsibility is assumed by Sponsor for any incorrect or missing contact information associated with an entry, or for any change of contact information of entrant after entry submission. Upon winner confirmation, Sponsor will provide information to winner on prize fulfillment.

PUBLICITY RELEASE: Except where prohibited, acceptance of the prize constitutes winner’s consent that his or her name, likeness, voice and/or biographical data may be used for advertising and promotional purposes without limitation and without additional notice, compensation, or consent.

RELEASE: By entering, entrant agrees to accept and abide by the Official Rules of this Sweepstakes and agrees that any dispute with regard to the conduct of this Sweepstakes, rule interpretation, or award of prize shall be submitted to Sponsor, whose decision shall be binding and final. By participating, entrant agrees to release and hold harmless Sponsor, participating dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (“Releasees”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to the acceptance or use/misuse of a prize, participation in any Sweepstakes-related activity, or participation in the Sweepstakes. Entrant hereby agrees to defend, indemnify, and hold harmless Releasees against any and all third-party claims, damages, liabilities, costs and expenses, including reasonable legal fees and expenses, arising out of or related to any breach by the entrant of any provision of this document, including, without limitation, entrant’s representations and warranties.

ADDITIONAL RULES: Sponsor is not responsible for any typographical or other error in the printing of this offer, administration of the Sweepstakes, or in the announcement of a prize, including erroneous appearance of qualification for the prize, and under no circumstances will more than the stated number of prizes be awarded. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Sweepstakes, or to amend the Official Rules at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Sweepstakes as contemplated by these Official Rules. If for any reason Sponsor is prevented from continuing with this Sweepstakes, or the integrity and/or feasibility of the Sweepstakes is severely undermined by any event including but not limited to fire, flood, epidemic, pandemic, earthquake, explosion, labor dispute or strike, act of God or public enemy, satellite or equipment failure, riot or civil disturbance, war (declared or undeclared), terrorist threat or activity, or any federal, state, or local government law, order, or regulation, or any order of any court or jurisdiction, Sponsor shall have the right, in its sole discretion, to abbreviate, modify, suspend, cancel, or terminate the Sweepstakes without further obligation. If Sponsor in its discretion elects to so abbreviate the Sweepstakes, Sponsor reserves the right, but not the obligation, to award the Prize from among all eligible entries received to date. All entries are the physical property of Sponsor and are not returnable.

DISPUTES / CHOICE OF LAW: This promotion is intended for viewing in the United States only and shall be construed and evaluated only according to applicable Wisconsin law. Do not enter this promotion if you are not located in Michigan, Ohio, Illinois, Indiana, OR Wisconsin. EXCEPT WHERE PROHIBITED, EACH ENTRANT AGREES THAT: (1) ANY AND ALL DISPUTES, CLAIMS AND CAUSES OF ACTION ARISING OUT OF OR CONNECTED WITH THIS SWEEPSTAKES OR ANY PRIZE AWARDED SHALL BE RESOLVED INDIVIDUALLY, WITHOUT RESORT TO ANY FORM OF CLASS ACTION, AND EXCLUSIVELY BY STATE OR FEDERAL COURTS SITUATED IN WISCONSIN; (2) ANY AND ALL CLAIMS, JUDGMENTS, AND AWARDS SHALL BE LIMITED TO ACTUAL OUT-OF-POCKET COSTS INCURRED, BUT IN NO EVENT MAY A CLAIM BE MADE FOR ATTORNEYS’ FEES; (3) NO PUNITIVE, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR OTHER DAMAGES, INCLUDING, WITHOUT LIMITATION, LOST PROFITS, MAY BE AWARDED (COLLECTIVELY, “SPECIAL DAMAGES”); AND (4) ENTRANT HEREBY WAIVES ALL RIGHTS TO CLAIM SPECIAL DAMAGES AND ALL RIGHTS TO SUCH DAMAGES MULTIPLIED OR INCREASED. WISCONSIN LAW, WITHOUT REFERENCE TO CHOICE OF LAW RULES, GOVERNS THE SWEEPSTAKES AND ALL ASPECTS RELATED THERETO.

Prize and Approximate Retail Value (“ARV”). 1 (1) entrant will be randomly selected from among all eligible entries. The potential winner (the “Winner”) will receive the following prize (“Prize”):

• The prize consists of a speaker.
• Total Approximate Retail Value (“ARV”) of Prize: $475.

Winning a prize is contingent upon fulfilling all requirements set forth herein. PLEASE NOTE: Taxes on prize are the sole responsibility of the winner. Affidavit of eligibility (including SSN), prize acceptance form, tax information, assignment of rights, background check, publicity release, and/or mutually acceptable liability releases may be required where not prohibited. Prizes are awarded without warranty of any kind from Sponsor, express or implied, without limitation, except where this would be contrary to federal, state, provincial, or local law or regulation and, in such case, the merchandise prizes are subject to the manufacturer’s and/or service providers’ terms and conditions, and the winners agree to look solely to such manufacturers and/or service providers for any warranty or guarantee claim. Gift cards, certificates, and vouchers may be subject to additional terms and conditions. Winners will be responsible for conducting themselves in accordance with venue terms and conditions. No substitution or cash equivalent is allowed except in Sponsor’s sole discretion. Prize is not transferable, except that any winner, regardless of age, may transfer prize to a parent or legal guardian. Sponsor reserves the right, but not the obligation, to substitute a prize of the same or equal approximate retail value. For prizes $600 or more in value to an individual winner in a calendar year, winner’s social security number or taxpayer ID will be required in order to issue a Form 1099-MISC showing prize ARV as income to winner. Prize consists only of items specifically listed as part of the prize. All costs and expenses not specified herein, related to any prize, are the sole responsibility of the winner.

PRIVACY: As a condition of entering the Sweepstakes, each entrant agrees that personally identifiable information submitted as part of an entry will be used to administer the Sweepstakes, select the winners, and fulfill the prize. Such information may be shared with Sponsor’s agents and affiliates as necessary to administer the Sweepstakes and to fulfill the prize. Information will be treated in accordance with Sponsor’s Privacy Policy available at www.versiti.org/privacy-policy.
