Versiti Blood Center of Indiana has partnered with the Indianapolis Colts to create awareness of the need for a strong and consistent blood supply within our community. By hosting a blood drive this summer, participating high schools can compete to receive a $1,000 equipment voucher from the Indianapolis Colts, among other prizes. More importantly, participating high schools can help their communities save lives.

The 2022 Versiti Blood Center of Indiana / Indianapolis Colts Leadership Challenge (the “Contest”) is sponsored by the Versiti Blood Center of Indiana, 3450 North Meridian Street, Indianapolis, IN 46208 (the “Sponsor”).

The Contest is not a game of chance; winning schools will be selected based on the criteria stated below. The Contest is subject to all federal, state, and local laws and regulations. Void where prohibited or restricted by law.

ELIGIBILITY: The Program is open to participating high schools located within the State of Indiana. Eligible high schools will fall into one of two categories:

1. “New Schools” include those high schools that did not host a Versiti blood drive as part of the 2021 Colts Leadership Challenge.
2. “Current Schools” include those high schools that hosted a Versiti blood drive as part of the 2021 Colts Leadership Challenge.

HOW TO PARTICIPATE: Eligible high schools interested in participating in the Contest should contact their Versiti Account Representative or contact Joe McGrath at the Versiti Blood Center of Indiana by phone (317-371-6307) or by email (jmcgrath@versiti.org). Once a high school notifies its representative or Joe McGrath of its desire to participate in the Contest, an affiliation code will be added to Versiti’s system to identify the blood drive associated with this Contest.

To participate in the Contest, a high school will be required to host one (1) blood drive between May 30, 2022 and September 5, 2022 (the “Contest Period”). Blood drives that take  place before or after the Contest Period will not be eligible. Sponsor will provide instructions and guidance to the high schools on planning, scheduling, and hosting a blood drive, including how to select a time and location for the blood drive and how to recruit volunteers and potential donors.

PRIZES AND APPROXIMATE RETAIL VALUES (“ARV”): Three (3) high schools will be selected as winners in total based on the criteria outlined below in Selection of Winners. Specifically, one (1) New School will be selected as a winner for collecting the most units as a New School, one (1) Current School or New School will be selected as a winner for collecting the most blood units, and  one (1) Current School will be selected as a winner for having the highest year-over-year increase in units collected (based on number of units, not percentage change). Each winning school will receive the following:

• $1,000 equipment voucher from the Indianapolis Colts (ARV: $1,000)
• NFL-regulation football customized with your high school’s team name and logo (ARV: $20; customization has no intrinsic value, priceless)
• Limited edition Colts Bleed Blue football (ARV: $20.00; customization has no intrinsic value, priceless)
• Public announcement to local press and coverage on Versiti’s and Colts social media channels (ARV: no intrinsic value, priceless)

Winning a prize is contingent upon fulfilling all requirements set forth herein. Taxes on prize (if any) are the sole responsibility of the winner. Where not prohibited, prize acceptance form, publicity release, and/or mutually acceptable liability releases may be required as a condition of prize award if requested by Sponsor. Any and all guarantees and warranties for merchandise prizes are subject to the manufacturer’s and/or service providers terms and conditions, and winner agrees to look solely to such manufacturers and/or service providers for any warranty or guarantee claim. Prizes are non-transferable except in the sole discretion of the Sponsor. No substitution or cash equivalent is allowed. Sponsor reserves the right to substitute prizes of the same approximate retail value. Prize consists only of items specifically listed as part of the prize. All costs and expenses not specified herein, related to any prize, including but not limited to delivery, installation, service activation fees, and other expenses incurred by accepting the prize, are the sole responsibility of the winner.

SELECTION OF WINNERS: Sponsor representatives will serve as the judges and will calculate the score for each participating school using the formula below:

The New School that achieves the highest school score from among all participating New Schools will be deemed a winner. The one Current School or New School that achieves the highest school score from among all participating Current Schools and New Schools will be deemed a winner. The Current School that achieves the highest year-over-year score will be deemed a winner. All judges’ and Sponsor’s decisions are final and binding. In the unlikely event of a tie affecting winner selection, the tied entrant that conducted its blood drive on the earlier date will be deemed the winner.

Please note that a single school may receive two prizes. Specifically, if “New School A” collects the most blood units among all New Schools (Category 1) and collects the most blood units among all New Schools and Current Schools (Category 2), then “New School A” will be selected as the winner of Category 1 and Category 2. Similarly, If “Current School B” collects the most blood units among all New Schools and Current Schools (Category 2) and achieves the highest year-over-year change in blood units (Category 3), then “Current School B” will be selected as the winner of Category 2 and Category 3.

NOTIFICATION OF WINNERS: Winning schools will be notified on or about September 16, 2022 by phone, email, and/or regular mail, in Sponsor’s sole discretion. Winner will be required to respond within fourteen (14) business days of notification, or the prize may be forfeited, and an alternate winner will be chosen. Prize acceptance forms and/or mutually acceptable publicity and liability releases may be required and, if so requested, must be returned within the reasonable specified time or prize will be forfeited and an alternate winner chosen. Upon winner confirmation, Sponsor will work with winning schools and provide information on how to obtain prize.

PUBLICITY RELEASE: Except where prohibited, acceptance of the prize constitutes winner’s consent that its name, likeness, and/or geographical data may be used for advertising and promotional purposes without limitation and without additional notice, compensation, or consent.

RELEASE: By entering, participating school agrees to accept and abide by the Official Rules of this Contest and agrees that any dispute with regard to the conduct of this Contest, rule interpretation, or award of prize shall be submitted to Sponsor, whose decision shall be binding and final. By participating, you release Sponsor, Indianapolis Colts, and their respective subsidiaries, affiliates, partners, employees, directors, and agents from any and against any and all injury, loss or damage including but not limited to, personal injury, death or damage to or loss of property, caused or claimed to be caused by your participation in the Contest. Entrant hereby agrees to defend, indemnify and hold harmless Sponsor, Indianapolis Colts, and their respective officers, directors, employees, agents, parent companies, affiliates, subsidiaries, licensees, successors, assignees, against any and all third party claims, damages, liabilities, costs and expenses, including reasonable legal fees and expenses, arising out of or related to any breach by the entrant of any provision of this document, including, without limitation, entrant’s representations and warranties.

THE NATIONAL FOOTBALL LEAGUE, ITS MEMBER PROFESSIONAL FOOTBALL CLUBS, NFL VENTURES, L.P., NFL PROPERTIES LLC, NFL ENTERPRISES LLC, NFL PRODUCTIONS LLC AND NFL INTERNATIONAL LLC AND EACH OF THEIR RESPECTIVE SUBSIDIARIES, AFFILIATES, SHAREHOLDERS, OFFICERS, DIRECTORS, AGENTS, REPRESENTATIVES, AND EMPLOYEES (COLLECTIVELY, THE “NFL ENTITIES”) WILL HAVE NO LIABILITY OR RESPONSIBILITY FOR ANY CLAIM ARISING IN CONNECTION WITH PARTICIPATION IN THIS CONTEST OR ANY PRIZE AWARDED. THE NFL ENTITIES, OTHER THAN THE INDIANAPOLIS COLTS, HAVE NO INVOLVEMENT IN THIS CONTEST IN ANY WAY.

ADDITIONAL RULES: Sponsor is not responsible for any typographical or other error in the printing of this offer, administration of the Contest, or in the announcement of prizes,
including erroneous appearance of qualification for a prize, and under no circumstances will more than the stated number of prizes be awarded. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Contest, or to amend the Official Rules at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Contest as contemplated by these Official Rules. If for any reason Sponsor is prevented from continuing with this Contest, or the integrity and/or feasibility of the Contest is severely undermined by any event including but not limited to fire, flood, epidemic, pandemic, earthquake, explosion, labor dispute or strike, act of God or public enemy, satellite or equipment failure, riot or civil disturbance, war (declared or undeclared), terrorist threat or activity, or any federal, state or local government law, order or regulation, order of any court or jurisdiction, Sponsor shall have the right, in its sole discretion, to abbreviate, modify, suspend, cancel or terminate the Contest without further obligation. If Sponsor in its discretion elects to so abbreviate the Contest, Sponsor reserves the right, but not the obligation, to award the prize from among all eligible participants to date.

DISPUTES / CHOICE OF LAW: EXCEPT WHERE PROHIBITED, EACH ENTRANT AGREES THAT: (1) ANY AND ALL DISPUTES, CLAIMS AND CAUSES OF ACTION ARISING OUT OF OR CONNECTED WITH THIS CONTEST OR ANY PRIZE AWARDED SHALL BE RESOLVED INDIVIDUALLY, WITHOUT RESORT TO ANY FORM OF CLASS ACTION, AND EXCLUSIVELY BY STATE OR FEDERAL COURTS SITUATED IN INDIANA, (2) ANY AND ALL CLAIMS, JUDGMENTS AND AWARDS SHALL BE LIMITED TO ACTUAL OUT-OF-POCKET COSTS INCURRED, BUT IN NO EVENT ATTORNEYS’ FEES, (3) NO PUNITIVE, INCIDENTAL, SPECIAL, CONSEQUENTIAL OR OTHER DAMAGES, INCLUDING, WITHOUT LIMITATION, LOST PROFITS, MAY BE AWARDED (COLLECTIVELY, “SPECIAL DAMAGES”), AND (4) ENTRANT HEREBY WAIVES ALL RIGHTS TO CLAIM SPECIAL DAMAGES AND ALL RIGHTS TO SUCH DAMAGES MULTIPLIED OR INCREASED. INDIANA LAW, WITHOUT REFERENCE TO CHOICE OF LAW RULES, GOVERNS THE CONTEST AND ALL ASPECTS RELATED THERETO.

PRIVACY: As a condition of entering the Contest, each entrant agrees that personally identifiable information submitted in connection with the Contest will be used to administer the Contest, select winners, and fulfill prizes. Such information may be shared with Sponsor’s agents and affiliates as necessary to administer the Contest and fulfill the prizes. Information will be treated in accordance with Sponsor’s Privacy Policy available at www.versiti.org/privacy-policy.

WINNER’S LIST: To receive any legally-required name of the winner and/or a copy of these Official Rules, send a legal-size, self-addressed, stamped envelope within sixty (60) days of the end of the Contest to: 2021 Versiti & Indianapolis Colts Leadership Challenge
