Our Navigator plays a key role in neighborhood health and well-being:
- Guides community members in navigating the healthcare system
- Connects individuals and families to vital resources
- Tracks referrals and appointments, including primary care, dental, and vision services
- Assists community members facing housing, safety, transportation, and food security challenges
- Provides personalized guidance on health priorities and healthcare needs
Additionally, the community health navigator plays a key role in promoting awareness about the critical importance of a vibrant and varied blood, organ, and tissue donation supply to ensure better health outcomes for everyone in our community.
