NO PURCHASE OR BLOOD DONATION NECESSARY TO ENTER OR WIN A PRIZE IN THIS SWEEPSTAKES. A PURCHASE OR DONATION WILL NOT IMPROVE YOUR CHANCES OF WINNING. VOID WHERE PROHIBITED BY LAW.

BY ENTERING (OR OTHERWISE PARTICIPATING) IN THE SWEEPSTAKES, YOU AGREE TO THESE OFFICIAL RULES, WHICH CREATE A CONTRACT, SO READ THEM CAREFULLY BEFORE ENTERING. WITHOUT LIMITATION, SUCH CONTRACT INCLUDES A CLASS ACTION WAIVER AND A LIMITATION OF YOUR RIGHTS AND REMEDIES.

SPONSOR: Versiti Blood Center of Indiana, 3450 N Meridian St. Indianapolis, IN 46208 (“Sponsor”).

ELIGIBILITY: The Versiti Blood Center of Indiana’s 2023 Concert Tickets Sweepstakes (the “Sweepstakes”) is open to legal U.S. residents of the State of Indiana who are 18 years of age or older at the time of entry. The following are not eligible: (i) employees, officers, directors, agents, independent contractors, and representatives of Sponsor and its respective parents, affiliates, subsidiaries, wholesalers, retailers, distributers, advertising, and promotion agencies; (ii) senior leadership of entities or organizations with contractual agreements in place with Versiti for blood-related products or services, as well as employees of such entities or organizations who had or have responsibility for negotiating such contractual agreements; and (iii) the immediate family members (mother, father, sister, brother, husband, wife, daughter, son, step-father, step-mother, step-daughter, step-son, step-brother, step-sister, half-brother, half-sister) and/or those living in the same household of any persons under (i) and (ii) above. The Sweepstakes is subject to all applicable federal, state, and local laws and regulations. Void where prohibited or restricted by law or regulations. Participation constitutes entrant’s full and unconditional agreement to these terms and conditions and Sponsor’s decisions, which are final and binding in all matters related to the Sweepstakes.

SWEEPSTAKES PERIOD: The Sweepstakes will take place between 12:01 a.m. Eastern Time (“ET”) on June 1, 2023, and 11:59 p.m. ET on June 30, 2023.

HOW TO ENTER: Eligible individuals may enter the Sweepstakes by one of two methods:

1. To enter by donating: Donate (or attempt to donate) blood at a mobile blood drive sponsored by the Versiti Blood Center of Indiana during the Sweepstakes Period. (For locations, please visit Versiti.org.) Donors will be tracked by Sponsor and will be automatically entered into the Sweepstakes after their donation. Specifically, eligible entrants who donate at a mobile drive during the Sweepstakes Period will receive one entry into the Sweepstakes. To be a donor, individuals must meet the minimum qualifications to donate. (Please visit Versiti.org for donor requirements.)
2. To enter without donating (“AMOE”): Eligible entrants may enter the Sweepstakes without a blood donation by doing the following: Visit Versiti.org/concert and fill out the entry form online during the Sweepstakes Period. By completing the online form, you will receive one entry into the Sweepstakes.

Limit one (1) entry per person for the Sweepstakes, regardless of method of entry. Any attempt by an entrant to obtain more than the stated number of entries by using multiple/different addresses or any other methods will void that entrant’s entries and that entrant may be disqualified from the Sweepstakes. Entries submitted by anyone other than the entrant are void. Sponsor will not verify receipt of entries for entrants. All entries submitted become the sole property of Sponsor and will not be returned. Sponsor is not responsible for lost, late, misdirected, illegible, or damaged entries.

SELECTION OF WINNERS, PRIZES, AND ODDS OF WINNING: One (1) winner will be randomly selected to receive the following:

• Four (4) lawn tickets to winner’s choice of one of four concerts (details below). (ARV: $300.00).

Winner will be given opportunity to select one of four concerts to attend: (i) Snoop Dogg, Wiz Khalifa, Too $hort, Warren G, and Berner, High School Reunion Tour, July, 20, 2023, Ruoff Music Center, Noblesville, IN; (ii) Luke Bryan: Country On Tour 2023, August 18, 2023, Ruoff Music Center, Noblesville, IN; (iii) Luke Bryan: Country On Tour 2023, August 25, 2023, Ford Center, Evansville, IN; or (iv) Pentatonix – The World Tour with Lauren Alaina, August 26, 2023, Ruoff Music Center, Noblesville, IN.

Odds of winning depend on the number of eligible entrants received for the Sweepstakes. A random drawing will be conducted on or about July 7, 2023, and potential winner will be notified on or about July 7, 2023 by phone or email, in Sponsor’s sole discretion, using the information provided at registration or on the online entry form. Winner will be required to respond within two (2) business days of notification, or the prize will be forfeited, and an alternate winner will be chosen. A prize acceptance form and/or mutually acceptable liability releases may be required and, if so requested by Sponsor, must be returned within the reasonable specified time or prize will be forfeited and an alternate winner chosen. Upon winner confirmation, Sponsor will provide information to winner on how to select and obtain the prize.

All taxes, fees, and surcharges on prizes are the sole responsibility of the winner. If the value of the prize exceeds $600, the Winner’s social security number or taxpayer I.D. will be required in order to issue a Form 1099-MISC showing Prize ARV as income to winner. Tickets subject to standard terms and conditions. Prizes are non-transferable except in the sole discretion of the Sponsor. No substitution or cash equivalent is allowed. Sponsor reserves the right to substitute prizes of the same approximate retail value. Prize consists only of items specifically listed as part of the prize. All costs and expenses not specified herein related to any prize or incurred by accepting the prize are the sole responsibility of the winner.

PUBLICITY RELEASE: Except where prohibited, acceptance of the prize constitutes winner’s consent that his or her name, likeness, voice and/or biographical data may be used for advertising and promotional purposes without limitation and without additional notice, compensation, or consent.

RELEASE: By entering, entrant agrees to accept and abide by the Official Rules of this Sweepstakes and agrees that any dispute with regard to the conduct of this Sweepstakes, rule interpretation, or award of prize shall be submitted to Sponsor, whose decision shall be binding and final. By participating, you release Sponsor, LiveNation, and their respective subsidiaries, affiliates, partners, employees, directors, and agents (collectively, “Released Parties”) from any and against any and all injury, loss, or damage including but not limited to, personal injury, death, or damage to or loss of property, caused or claimed to be caused by entrant’s participation in the Sweepstakes. Entrant hereby agrees to defend, indemnify and hold harmless the Released Parties and their respective officers, directors, employees, agents, parent companies, affiliates, subsidiaries, licensees, successors, and assignees, against any and all third party claims, damages, liabilities, costs, and expenses, including reasonable legal fees and expenses, arising out of or related to entrant’s participation in the Sweepstakes or any breach by entrant of any provision of this document, including, without limitation, entrant’s representations and warranties.

ADDITIONAL RULES: Sponsor is not responsible for any typographical or other error in the printing of this offer, administration of the Sweepstakes, or in the announcement of the prize, including erroneous appearance of qualification for the prize, and under no circumstances will more than the stated number of prizes be awarded. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Sweepstakes, or to amend the Official Rules at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Sweepstakes as contemplated by these Official Rules. If for any reason Sponsor is prevented from continuing with this Sweepstakes, or the integrity and/or feasibility of the Sweepstakes is severely undermined by any event including but not limited to fire, flood, epidemic, earthquake, pandemic, explosion, labor dispute or strike, act of God or public enemy, satellite or equipment failure, riot or civil disturbance, war (declared or undeclared), terrorist threat or activity, or any federal, state or local government law, order or regulation, order of any court or jurisdiction, Sponsor shall have the right, in its sole discretion, to abbreviate, modify, suspend, cancel, or terminate the Sweepstakes without further obligation. If Sponsor in its discretion elects to so abbreviate the Sweepstakes, Sponsor reserves the right, but not the obligation, to award the Prize from among all eligible entries received to date. All entries are the physical property of Sponsor and are not returnable.

DISPUTES / CHOICE OF LAW: EACH ENTRANT AGREES THAT (1) ANY AND ALL DISPUTES, CLAIMS, AND CAUSES OF ACTION ARISING OUT OF OR IN CONNECTION WITH THIS SWEEPSTAKES, OR ANY PRIZE AWARDED, SHALL BE RESOLVED INDIVIDUALLY, WITHOUT RESORT TO ANY FORM OF CLASS ACTION, AND ANY JUDICIAL PROCEEDING SHALL TAKE PLACE IN A FEDERAL OR STATE COURT IN INDIANA; (2) ANY AND ALL CLAIMS, JUDGMENTS, AND AWARDS SHALL BE LIMITED TO ACTUAL OUT-OF-POCKET COSTS INCURRED, INCLUDING COSTS ASSOCIATED WITH ENTERING THIS SWEEPSTAKES, BUT IN NO EVENT ATTORNEYS’ FEES; AND (3) UNDER NO CIRCUMSTANCES WILL ENTRANT BE PERMITTED TO OBTAIN AWARDS FOR, AND ENTRANT HEREBY WAIVES ALL RIGHTS TO CLAIM, PUNITIVE, INCIDENTAL, AND CONSEQUENTIAL DAMAGES AND ANY OTHER DAMAGES, OTHER THAN FOR ACTUAL OUT-OF-POCKET EXPENSES, AND ANY AND ALL RIGHTS TO HAVE DAMAGES MULTIPLIED OR OTHERWISE INCREASED. ALL ISSUES AND QUESTIONS CONCERNING THE CONSTRUCTION, VALIDITY, INTERPRETATION AND ENFORCEABILITY OF THESE OFFICIAL RULES, OR THE RIGHTS AND OBLIGATIONS OF ENTRANT AND THE SPONSOR IN CONNECTION WITH THIS SWEEPSTAKES SHALL BE GOVERNED BY, AND CONSTRUED IN ACCORDANCE WITH THE LAWS OF THE STATE OF INDIANA, WITHOUT GIVING EFFECT TO ANY CHOICE OF LAW OR CONFLICT OF LAW RULES OR PROVISIONS OF ANY OTHER JURISDICTION THAT WOULD CAUSE THE APPLICATION OF THE LAWS OF ANY JURISDICTION OTHER THAN THOSE OF THE STATE OF INDIANA.

PRIVACY: As a condition of entering the Sweepstakes, each entrant agrees that personally identifiable information submitted as part of an entry will be used to administer the Sweepstakes, select winners, and fulfill prizes. Such information may be shared with Sponsor’s agents and affiliates, and business relations, including LiveNation, as necessary to administer the Sweepstakes and fulfill the prize. Information will be treated in accordance with Sponsor’s Privacy Policy available at www.versiti.org/privacy-policy.
