• BY ENTERING (OR OTHERWISE PARTICIPATING) IN THE SWEEPSTAKES, YOU AGREE TO THESE OFFICIAL RULES, WHICH CREATE A CONTRACT, SO READ THEM CAREFULLY BEFORE ENTERING. WITHOUT LIMITATION, SUCH CONTRACT INCLUDES A CLASS ACTION WAIVER AND A LIMITATION OF YOUR RIGHTS AND REMEDIES. 

SPONSOR: Versiti, Inc., 638 North 18th Street, Milwaukee, WI 53233. (“Sponsor”).

ELIGIBILITY: The 2024 Versiti Blood Center America’s Superstar Sweepstakes (the “Sweepstakes”) is open to legal U.S. residents of Michigan, Ohio, Illinois, Indiana, and Wisconsin who are 18 years of age or older at the time of entry and have reached the age of majority in their respective states of residence. The following are not eligible: (i) employees, officers, directors, agents, independent contractors, and representatives of Sponsor and its respective parents, affiliates, subsidiaries, wholesalers, retailers, distributers, advertising, and promotion agencies; (ii) senior leadership of entities or organizations with contractual agreements in place with Versiti for blood-related products or services, as well as employees of such entities or organizations who had or have responsibility for negotiating such contractual agreements; and (iii) the immediate family members (mother, father, sister, brother, husband, wife, daughter, son, step-father, step-mother, step-daughter, step-son, step-brother, step-sister, half-brother, half-sister) and/or those living in the same household of any persons under (i) and (ii) above. The Sweepstakes is subject to all applicable federal, state, and local laws and regulations. Void where prohibited or restricted by law or regulations. Participation constitutes entrant’s full and unconditional agreement to these terms and conditions and Sponsor’s decisions, which are final and binding in all matters related to the Sweepstakes.

SWEEPSTAKES PERIOD: The Sweepstakes will take place from September 16, 2024 to October 16, 2024 (“Sweepstakes Period”).

HOW TO ENTRY: Eligible individuals may enter Sweepstakes by one of two methods:

1. To enter by donating: Donate or attempt to donate at a Versiti Blood Center donor center or mobile blood drive located in Wisconsin, Michigan, Ohio, Indiana, or Illinois during the Sweepstakes Period. (For locations, please visit Versiti.org.) Donors will be tracked by Sponsor and will be automatically entered into the Sweepstakes after their donation. Specifically, eligible entrants who donate during the Sweepstakes Period will receive one (1) entry into the Sweepstakes. To be a donor, individuals must meet the minimum qualifications to donate. (Please visit Versiti.org for donor requirements.)

(2) To enter without donating (“AMOE”): Eligible entrants may enter the Sweepstakes without a blood donation by doing the following: Visit Versiti.org/AmericasSuperstar and fill out the entry form online during the Sweepstakes Period. By completing the online form, you will receive one entry into the Sweepstakes.

Limit one (1) entry per person during the Sweepstakes Period using one of the two methods listed above. The choice of entry method will not affect an entrant’s chances of winning. Any attempt by an entrant to obtain more than the stated number of entries by using multiple/different email addresses, and /or any other methods will void that entrant’s entries and that entrant may be disqualified from the Sweepstakes. Entries submitted by anyone other than the entrant are void. Sponsor will not verify receipt of entries for entrants. All entries submitted become the sole property of Sponsor and will not be returned. Sponsor is not responsible for lost, late, misdirected, illegible, or damaged entries.

Prizes and Approximate Retail Value (“ARV”): One prize will be randomly awarded in total. The prize consists of the following:

• Two (2) floor tickets to Taylor Swift’s Eras Tour at Lucas Oil Stadium on November 1, 2024 and a $500 visa gift card to assist with related expenses. (ARV: $3,500.)

Winning a prize is contingent upon fulfilling all requirements set forth herein. PLEASE NOTE: All taxes on prizes are the sole responsibility of the winner. Any and all guarantees and warranties for merchandise prizes are subject to the manufacturer’s and/or service providers’ terms and conditions, and winner agrees to look solely to such manufacturers and/or service providers for any warranty or guarantee claim. Where not prohibited, affidavit of eligibility (including SSN), prize acceptance form, tax information, publicity release, and/or mutually acceptable liability releases may be required from winner. Prize is not transferable and no substitution or cash equivalent is allowed, except in Sponsor’s sole discretion. Sponsor reserves the right to substitute prize of the same approximate retail value if advertised prize becomes unavailable for any reason. If the value of the prize exceeds $600, the winner’s social security number or taxpayer I.D. will be required in order to issue a Form 1099-MISC showing prize ARV as income to winner. Prize consists only of items specifically listed as part of the prize. All costs and expenses not specified herein relating to the prize are the sole responsibility of the winner.

PLEASE NOTE: Neither Taylor Swift nor entities affiliated with Taylor Swift are a sponsor of this Sweepstakes. The names of individuals, groups, companies, products, and services mentioned herein, and any corresponding likenesses, logos, and images thereof reproduced herein, have been used for identification purposes only and/or with permission, and may be the copyrighted properties and trademarks of their respective owners. The mention of any individual, group, or company, or the inclusion of a product or service as a prize, does not imply any association with or endorsement by such individual, group, or company or the manufacturer or distributor of such product or service and, except as otherwise indicated, no association or endorsement is intended or should be inferred.

SELECTION OF WINNERS AND ODDS OF WINNING: Odds of winning depend on the number of eligible entries received during the Sweepstakes Period. Potential prize winner will be selected in a random drawing from among all eligible entries on or about October 18, 2024. Drawings will be conducted by Sponsor in accordance with the Official Rules, whose decisions on all matters related to the Sweepstakes are binding and final. The potential prize winner will be notified via the contact information provided by the participant at the time of entry. If any winner notification remains unanswered for more than seventy-two (72) hours, the prize will be automatically forfeited in its entirety and an alternate winner will be selected. Sponsor is not responsible for false, incorrect, changed, incomplete, or illegible contact information or for failure of a potential winner to contact Sponsor after being selected as a winner. A potential winner unwilling to accept the prize, or to otherwise agree to the terms of the prize award, will be disqualified, and an alternate potential winner will be chosen in a random drawing. Upon responding to Sponsor’s notification, Sponsor will provide winner with instructions on prize fulfillment.

PUBLICITY RELEASE: Except where prohibited, acceptance of the prize constitutes winner’s consent that his or her name, likeness, voice, and/or biographical data may be used for advertising and promotional purposes without limitation and without additional notice, compensation, or consent.

ENTRANTS’ AGREEMENT TO ABIDE BY OFFICIAL RULES: By entering, entrant agrees that s/he has read, accepted, and will abide by and be bound by these Official Rules and agrees that any dispute with regard to the conduct of this Sweepstakes, rule interpretation, or the awarding of the prize shall be submitted to Sponsor, whose decision shall be binding and final.

RELEASE OF LIABILITY: By entering, entrant agrees to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, agencies, wholesalers, distributors and promotional and other vendor agencies involved in the Sweepstakes, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (“Releasees”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to the acceptance or use/misuse of the prize, participation in any Sweepstakes-related activity, or participation in the Sweepstakes.

LIMITATION OF LIABILITY: Releasees are not responsible for, and each entrant releases and holds harmless the Releasees from and against, any and all losses, damages, actions, demands, liabilities, or claims of whatever nature or kind arising out of, or in connection with a prize or any portion thereof or this Sweepstakes. Without limiting the generality of the foregoing, the Releasees will not be responsible for any typographical or other error in the printed or other related material related of this Sweepstakes, the administration of the Sweepstakes, the selection of entrants, in the announcement of the prize, including erroneous appearance of qualification for the prize, any entries or entrants disqualified for any of the reasons stated in these Official Rules, or any loss, damage or claims caused by, or in any way related to the prize or the Sweepstakes itself.

WARNING: ANY ATTEMPT BY ANY PERSON TO DELIBERATELY UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES MAY BE A VIOLATION OF CRIMINAL AND/OR CIVIL LAWS. SHOULD SUCH ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK REMEDIES AND DAMAGE FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW AND TO BAN OR DISQUALIFY SUCH PERSON FROM THIS AND FUTURE SWEEPSTAKES.

DISPUTES REGARDING THE IDENTITY OF ENTRANT: Entries will be deemed made by the stated blood donor or the authorized email account holder of the entry submitted. In the event of a dispute, the selected entrant may be required to provide proof that s/he is the authorized account holder of the identified account.

CHANGES TO SWEEPSTAKES, OFFICIAL RULES, ETC.: Under no circumstances will more than the stated number of prizes be awarded. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Sweepstakes, or to amend the Official Rules at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Sweepstakes as contemplated by these Official Rules. If for any reason Sponsor is prevented from continuing with this Sweepstakes, or the integrity and/or feasibility of the Sweepstakes is undermined by any event including but not limited to fire, flood, pandemic, epidemic, earthquake, explosion, labor dispute or strike, act of God or public enemy, satellite or equipment failure, riot or civil disturbance, war (declared or undeclared), terrorist threat or activity, any federal, state, provincial/territorial, or local government law, order, or regulation, or any order of any court or jurisdiction, Sponsor shall have the right, but not the obligation, in its sole and absolute discretion, to award the Prize from among all entries received to date.

DISPUTES/CHOICE OF LAW: THIS PROMOTION IS INTENDED FOR VIEWING IN THE UNITED STATES ONLY AND SHALL BE CONSTRUED AND EVALUATED ONLY ACCORDING TO APPLICABLE WISCONSIN LAW. DO NOT ENTER THIS PROMOTION IF YOU ARE NOT LOCATED IN MICHIGAN, OHIO, ILLINOIS, INDIANA, OR WISCONSIN. EXCEPT WHERE PROHIBITED, EACH ENTRANT AGREES THAT: (1) ANY AND ALL DISPUTES, CLAIMS AND CAUSES OF ACTION ARISING OUT OF OR CONNECTED WITH THIS SWEEPSTAKES OR ANY PRIZE AWARDED SHALL BE RESOLVED INDIVIDUALLY, WITHOUT RESORT TO ANY FORM OF CLASS ACTION, AND EXCLUSIVELY BY STATE OR FEDERAL COURTS SITUATED IN WISCONSIN; (2) ANY AND ALL CLAIMS, JUDGMENTS, AND AWARDS SHALL BE LIMITED TO ACTUAL OUT-OF-POCKET COSTS INCURRED, BUT IN NO EVENT MAY A CLAIM BE MADE FOR ATTORNEYS’ FEES; (3) NO PUNITIVE, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR OTHER DAMAGES, INCLUDING, WITHOUT LIMITATION, LOST PROFITS, MAY BE AWARDED (COLLECTIVELY, “SPECIAL DAMAGES”); AND (4) ENTRANT HEREBY WAIVES ALL RIGHTS TO CLAIM SPECIAL DAMAGES AND ALL RIGHTS TO SUCH DAMAGES MULTIPLIED OR INCREASED. WISCONSIN LAW, WITHOUT REFERENCE TO CHOICE OF LAW RULES, GOVERNS THE SWEEPSTAKES AND ALL ASPECTS RELATED THERETO.

RULES GOVERN: The Sweepstakes will be run in accordance with these Official Rules, which shall prevail, govern, and control in the event of any inconsistency with any Sweepstakes-related materials.

PRIVACY: Personally identifiable information that is submitted by entrants as part of this Sweepstakes will be used to administer the Sweepstakes, select the Prize winner, and fulfill the Prizes, and will be treated in accordance with Sponsor’s Privacy Policy available at www.versiti.org/privacy-policy.

WINNERS’ LIST: To receive any legally required name of the winner and/or a copy of these Official Rules, send a legal-size, self-addressed, stamped envelope within ninety (90) days of the end of the Sweepstakes to: 2024 Versiti Blood Center – America’s Superstar Sweepstakes–Winner’s List, Attn: Donor Marketing, P.O. Box 2178, Milwaukee, WI 53201- 2178. Winners’ names may also be announced via the Versiti website at www.versiti.org.
