• BY ENTERING (OR OTHERWISE PARTICIPATING) IN THE SWEEPSTAKES, YOU AGREE TO THESE OFFICIAL RULES, WHICH CREATE A CONTRACT, SO READ THEM CAREFULLY BEFORE ENTERING. WITHOUT LIMITATION, SUCH CONTRACT INCLUDES A CLASS ACTION WAIVER AND A LIMITATION OF YOUR RIGHTS AND REMEDIES. 

SPONSOR: Versiti Indiana, Inc., 3450 N. Meridian St., Indianapolis, Indiana 46208 (“Sponsor”).

ELIGIBILITY: Versiti’s “Fever Fridays” (“Sweepstakes”) is open only to legal U.S. residents of Indiana who are 18 years of age or older at the time of entry and have reached the age of majority in their respective states of residence. The following are not eligible: (i) employees, officers, directors, agents, independent contractors, and representatives of Sponsor, Indiana Fever Basketball, LLC (“Fever”), the Women’s National Basketball Association and its Teams, WNBA Enterprises, LLC and each of their respective parents, affiliates, subsidiaries, wholesalers, retailers, distributers, advertising, and promotion agencies; (ii) employees, officers, directors, agents, or representatives associated with entities that prohibit the acceptance of items from Versiti involving amounts equal to the approximate retail values of the prizes identified below; and (iii) the immediate family members (mother, father, sister, brother, husband, wife, daughter, son, step-father, step-mother, step-daughter, step-son, step-brother, step-sister, half-brother, half-sister) and/or those living in the same household of any persons under (i) and (ii) above. The Sweepstakes is subject to all applicable federal, state, and local laws and regulations. Void where prohibited or restricted by law or regulations. Participation constitutes entrant’s full and unconditional agreement to these terms and conditions and Sponsor’s decisions, which are final and binding in all matters related to the Sweepstakes.

SWEEPSTAKES PERIOD: The Sweepstakes consists of nine (9) entry periods and drawings which will take place on the dates listed below. Entries from one period will NOT roll-over to subsequent periods nor drawings.

HOW TO ENTRY: Eligible individuals may enter the Sweepstakes by one of two methods:

1. To enter by donating: To receive one (1) entry into the Sweepstakes, donate blood or attempt to donate blood at any Versiti Blood Center of Indiana Donor Center or mobile drive during the applicable Entry Period (listed in the table above). The donor will receive one entry into the drawing associated with the entry period. To be a blood donor, individuals must meet the minimum qualifications to donate. (Please visit Versiti.org for donor requirements.)

(2) To enter without donating (“AMOE”): Eligible entrants may enter the Sweepstakes without a blood donation by doing the following: Visit Versiti.org/FeverFridays during an entry period during the Sweepstakes Period (listed in the table above), and fill out the entry form online. By completing the online form, you will receive one entry into the drawing associated with the entry date.

Limit one (1) entry per person per drawing. Non-winning entries into one drawing will not roll-over to subsequent drawings; if an eligible individual is interested in entering subsequent drawings, he/she must enter again on a subsequent entry date using one of the two listed entry methods. The choice of entry method will not affect an entrant’s chances of winning. Any attempt by an entrant to obtain more than the stated number of entries by using multiple/different email addresses, and /or any other methods will void that entrant’s entries and that entrant may be disqualified from the Sweepstakes. Entries submitted by anyone other than the entrant are void. Sponsor will not verify receipt of entries for entrants. All entries submitted become the sole property of Sponsor and will not be returned. Sponsor is not responsible for lost, late, misdirected, illegible, or damaged entries.

Prizes and Approximate Retail Value (“ARV”): Two winners will be randomly selected for each of the nine separate drawings. The selected winners for each drawing (listed in the table above) will receive the prizes listed below:

Winning a prize is contingent upon fulfilling all requirements set forth herein. PLEASE NOTE: All taxes on prizes are the sole responsibility of the winner. Any and all guarantees and warranties for merchandise or services prizes are subject to the manufacturer’s and/or service provider’s terms and conditions, and winner agrees to look solely to such manufacturers and/or service providers for any warranty or guarantee claim. Where not prohibited, affidavit of eligibility (including SSN), prize acceptance form, tax information, publicity release, and/or mutually acceptable liability releases may be required from winner. Prize is not transferable and no substitution or cash equivalent is allowed, except in Sponsor’s sole discretion. Sponsor reserves the right to substitute prize of the same approximate retail value if advertised prize becomes unavailable for any reason. If the value of the prize is equal to or exceeds $600, the winner’s social security number or taxpayer I.D. will be required in order to issue a Form 1099-MISC showing prize ARV as income to winner. Prize consists only of items specifically listed as part of the prize. All costs and expenses not specified herein relating to the prize are the sole responsibility of the winner.

Prize winner and his/her guest must agree to comply with all WNBA venue rules and regulations relating to the use of WNBA regular and post-season tickets (as applicable) and, in accordance therewith, agree not to act in an unsportsmanlike or disruptive manner, or with any intent to annoy, abuse, threaten or harass any other person at such game.  Failure to do so may result in forfeiture of prize or prize element.

ODDS OF WINNING, WINNER SELECTION, AND NOTIFICATION: Odds of winning depend on the number of eligible entries received. The potential winner will be selected randomly on or about May 9, 2025, May 23, 2025, June 13, 2025, June 20, 2025, July 7, 2025, July 11, 2025, August 8, 2025, August 15, 2025, August 29, 2025. Winner will be notified via phone and/or email on or about May 9, 2025, May 23, 2025, June 13, 2025, June 20, 2025, July 7, 2025, July 11, 2025, August 8, 2025, August 15, 2025, August 29, 2025 (using information in Sponsor’s records or information provided via the mail-in entry) to verify his/her eligibility and ability to accept the prize, and to obtain his/her complete contact information. Any such message will instruct the winner to reply as specified within five (5) full business days (measured in the local time zone) after the time that the message was left or sent. Winner must reply as instructed to identify him/herself, confirm his/her eligibility and ability to accept the prize, and provide his/her complete contact information within such five (5) full business day period, or he/she will be disqualified. Potential winner will subsequently be required to complete an affidavit/certification of eligibility, a liability release, and (where legal) a publicity release, which must be returned within the reasonable time period specified by Sponsor at the time of such request. Winner will also be required to return a properly completed IRS W-9 form within the time period specified by Sponsor, and each such winner will receive an IRS Form 1099 issued in his/her name for the fair market value of prize received if prize meets or exceeds ARV of $600. Proof of identity, age, and legal residence may be required to claim a prize. Winner may be disqualified in Sponsor’s sole discretion if: (a) he/she fails to reply or to provide any required information or properly completed documents within the required time period; (b) he/she fails to comply with any of the terms of these Official Rules; or (c) if any prize or prize notification is returned as undeliverable. In the event of disqualification of the winner, at Sponsor’s sole discretion the prize may be awarded to an alternate winner selected from among the remaining applicable eligible entries via the means of winner determination described herein, pending verification of eligibility. If any prize remains unclaimed after three (3) potential winners have been notified for that prize, the prize may remain unawarded, in Sponsor’s sole discretion. No responsibility is assumed by Sponsor or Releasees for any incorrect or missing contact information associated with an entry, or for any change of contact information of entrant after entry submission. Upon winner confirmation, Sponsor will provide information to winner on prize fulfillment.

PUBLICITY RELEASE: As a condition of entering the Sweepstakes, each entrant agrees that personally identifiable information submitted as part of an entry will be used to administer the Sweepstakes, select the winners, and fulfill the prize. Such information may be shared with Sponsor’s agents and affiliates as necessary to administer the Sweepstakes and to fulfill the prize. Information will be treated in accordance with Sponsor’s Privacy Policy available at www.versiti.org/privacy-policy.

ENTRANTS’ AGREEMENT TO ABIDE BY OFFICIAL RULES: By entering, entrant agrees that s/he has read, accepted, and will abide by and be bound by these Official Rules and agrees that any dispute with regard to the conduct of this Sweepstakes, rule interpretation, or the awarding of the prize shall be submitted to Sponsor, whose decision shall be binding and final.

RELEASE: By entering, entrant agrees to accept and abide by the Official Rules of this Sweepstakes and agrees that any dispute with regard to the conduct of this Sweepstakes, rule interpretation, or award of prize shall be submitted to Sponsor, whose decision shall be binding and final. By participating, entrant agrees to release and hold harmless Sponsor, Indiana Fever, the Women’s National Basketball Association and its Teams, WNBA Enterprises, LLC, participating dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, and each of their respective parent companies, affiliates, subsidiaries, related entities, service agencies, and independent contractors, as well as the officers, directors, owners, shareholders, partners, members, employees, agents and representatives, successors, and assigns of any of the above organizations (“Releasees”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to the acceptance or use/misuse of a prize, participation in any Sweepstakes-related activity, or participation in the Sweepstakes. Entrant hereby agrees to defend, indemnify, and hold harmless Releasees and their insurers against any and all third-party claims, damages, liabilities, costs and expenses, including reasonable legal fees and expenses, arising out of or related to (i) any breach by the entrant of any provision of this document, including, without limitation, entrant’s representations and warranties, and (ii) the entrant’s acceptance or use/misuse of a prize, participation in any Sweepstakes-related activity, or participation in the Sweepstakes.

LIMITATION OF LIABILITY: Releasees are not responsible for, and each entrant releases and holds harmless the Releasees from and against, any and all losses, damages, actions, demands, liabilities, or claims of whatever nature or kind arising out of, or in connection with a prize or any portion thereof or this Sweepstakes. Without limiting the generality of the foregoing, the Releasees will not be responsible for any typographical or other error in the printed or other related material related of this Sweepstakes, the administration of the Sweepstakes, the selection of entrants, in the announcement of the prize, including erroneous appearance of qualification for the prize, any entries or entrants disqualified for any of the reasons stated in these Official Rules, or any loss, damage or claims caused by, or in any way related to the prize or the Sweepstakes itself.

WARNING: ANY ATTEMPT BY ANY PERSON TO DELIBERATELY UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES MAY BE A VIOLATION OF CRIMINAL AND/OR CIVIL LAWS. SHOULD SUCH ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK REMEDIES AND DAMAGE FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW AND TO BAN OR DISQUALIFY SUCH PERSON FROM THIS AND FUTURE SWEEPSTAKES.

ADDITIONAL RULES: Releasees are not responsible for any typographical or other error in the printing of this offer, administration of the Sweepstakes, or in the announcement of a prize, including erroneous appearance of qualification for the prize, and under no circumstances will more than the stated number of prizes be awarded. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Sweepstakes, or to amend the Official Rules at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Sweepstakes as contemplated by these Official Rules. If for any reason Sponsor is prevented from continuing with this Sweepstakes, or the integrity and/or feasibility of the Sweepstakes is severely undermined by any event including but not limited to fire, flood, epidemic, pandemic, earthquake, explosion, labor dispute or strike, act of God or public enemy, satellite or equipment failure, riot or civil disturbance, war (declared or undeclared), terrorist threat or activity, or any federal, state, or local government law, order, or regulation, or any order of any court or jurisdiction, Sponsor shall have the right, in its sole discretion, to abbreviate, modify, suspend, cancel, or terminate the Sweepstakes without further obligation. If Sponsor in its discretion elects to so abbreviate the Sweepstakes, Sponsor reserves the right, but not the obligation, to award the Prize from among all eligible entries received to date. All entries are the physical property of Sponsor and are not returnable.

DISPUTES / CHOICE OF LAW: Except where prohibited, each entrant agrees that: (1) any and all disputes, claims and causes of action arising out of or connected with this SWEEPSTAKES or any Prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by state or federal courts situated in INDIANA, (2) any and all claims, judgments, and awards shall be limited to actual out-of-pocket costs incurred, but in no event attorneys’ fees, (3) no punitive, incidental, special, consequential, or other damages, including, without limitation, lost profits, may be awarded (collectively, “Special Damages”), and (4) entrant hereby waives all rights to claim Special Damages and all rights to such damages multiplied or increased. INDIANA law, without reference to choice of law rules, governs the SWEEPSTAKES and all aspects related thereto.

CHANGES TO SWEEPSTAKES, OFFICIAL RULES, ETC.: Under no circumstances will more than the stated number of prizes be awarded. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Sweepstakes, or to amend the Official Rules at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Sweepstakes as contemplated by these Official Rules. 

RULES GOVERN: The Sweepstakes will be run in accordance with these Official Rules, which shall prevail, govern, and control in the event of any inconsistency with any Sweepstakes-related materials.

PRIVACY: Personally identifiable information that is submitted by entrants as part of this Sweepstakes will be used to administer the Sweepstakes, select the Prize winner, and fulfill the Prizes, and will be treated in accordance with Sponsor’s Privacy Policy available at www.versiti.org/privacy-policy.
